COVID Protocols for Stratford Arena Facilities
Submitted By Jessica Traynor on Wednesday, September 1, 2021
Please see the message below from the City of Stratford Community Services Department regarding COVID-19 screening.

Please be advised that we have implemented an online COVID-19 active screening tool to be used when entering the Rotary Complex, Allman Arena, and Dufferin Arena.  QR codes will be displayed at the entrances which will allow everyone that enters the facility to complete the screening online before entering the facility.  This would be completed in place of the paper copy COVID-19 screening forms.
If you are unable to complete the online screening forms, the paper copy forms will still be available.

The confirmation must be presented to the facility staff that is working the screening table before entering the facility.

This is the link for the online screening form.  https://forms.stratford.ca/COVID-19-Protocols/COVID-19-Screening-Questionnaire

Please note that online screening is required each time you enter a facility.    Also as a reminder - Participants are to arrive no more than 15 minutes before their rental time
