As SMHA prepares for the 2018-2019 season, we invite parents to participate in our Parent Survey.
This survey assists SMHA in making educated decisions for the association, and assists the Coaches Selection Committee in making decisions regarding future coaches.
We ask that parents complete one survey for each child they have in the SMHA system. Only one survey per player please. Deadline for participation is January 31st. We look forward to hearing from you and appreciate you taking the time to complete it — it should only take a few minutes.
Surveys are strictly confidential; the data is compiled using a third party program and given to the Coaches Selection Committee in a simple table format with all answers and comments, but no identifiers.
