Please ensure that you have completed the City of Stratford Waiver. You will not be allowed on the ice until this form is completed.
The Stratford Minor Hockey Health Screening Questionnaire must be reviewed prior to each ice time. You are required to answer these questions before you will be allowed on the ice.
